I understand that this deduction is after tax and will stop on the last pay period in October in year that the form was signed.
I understand that this money will be paid each year the first payroll in November in year in which the form was signed. It will be put back on your payroll check, not an AP check.
This money will not accrue any interest and will not be paid before November of the year signed.
You will need to submit a new form each year in January and the deduction will begin by the first payroll of January of that year or as soon as it is received by payroll.
This deduction or club can be stopped by the Business Manager at any time. You may cancel this deduction at any time, but money will not be paid before November. If you leave employment, then will be paid on your final check.
By signing this form, you agree to all terms of this savings club.
